This booking can now be made via the Payments area of the Reach More Parents (Weduc) app.

Change of Details Form - Please complete and return to school a Change of Details form if any contact details change throughout the school year.

Leave of Absence Form - A Leave of Absence form is to be completed to request absence from school throughout the school day including medical/dental appointments.

Consent to Give Medicine in School Form - A Medicine in School form needs to be completed for any medication that needs to be given to your child during school and must be prescribed.

Medical Information Form (Individual Healthcare Plan) - A Medical Information Form (Individual Healthcare Plan) is to be completed if your child has a medical condition or allergy that they will need support with in school.

School Shop Order Form - To order extra uniform items (book bags, hats etc) please fill in the School Shop Order Form and bring to the School Office, we require exact money please.
